Housing
- 81% of tenants remained permanently housed for 12 months (65% goal).
- 54% of tenants remained permanently housed for 24 months* (65% goal).
- 90% of tenants were compliant with their monthly social service plans (60% goal).
*76% of clients that left reached self-sufficiency, meaning they are now paying market rent.
Income & Skill Improvement
- 59% of tenants maintained or increased their income through acquisition of benefits, employment, or both (55% goal).
- Clients exiting the program (16), on average, had a 170% increase in income.
- 90% of individuals/families received one or more referral(s) for wrap around services (90% goal).
- 75% of residents attended 3 or more self-help meetings weekly (60% goal).
- 55% of tenants referred for job training and GED – successfully completed the course (50% goal).
Compliance & Health
- 54% of tenants attended quarterly town hall meetings (60% goal).
- 67% of residents referred for mental and physical health services were linked and received services (50% goal).
- 62% of individuals referred for substance abuse treatment completed treatment (25% goal).
Shelter Graduates – 29 Family Scattered Site Apartments
- 100% of families in the program retained their housing, 6 families left the program, but remained in permanent housing at fair market rent.
- On-going social service assessments were achieved with 80 staffings throughout the year. These staffings work to assess the family’s current status and to plan future goals.
- 20 rental subsidies were maintained. We also expanded our reach to an additional 9 rental subsidies, bringing our total to 29.
- Family plans were developed monthly and monitored for all families in the program.
- 792 case management contacts were made for the year to assist in housing retention services and improve family relations.
- 70 referrals to mainstream social, medical, employment, educational, financial, family support, legal, and child care services were made by the staff, 17 were made by the nurses. Nurses made contacts with 111 people in the program.
- 653 home contacts were made by case managers and 54 contacts were made in person.
- 20 referrals were made by staff and 7 were made by nurses to provide medical support services, on-site health screenings, nutritional education, and medical referrals.
- Nurses monitored the immunization status of 43 school age children (under the age of 12).
- 4 clients gained new employment.
- 7 completed job training.
- 11 were employed during the year.
- 5 have maintained personal savings.
- 1 family was reunited.
- 2 children were returned from DCFS.
